II. Purpose of This Questionnaire
This questionnaire aims to help you:

III. Questionnaire Content and Scoring Method
This questionnaire contains 13 questions. Each question describes an emotional, thinking, or behavioral state. Please answer based on your actual feelings over the past two weeks. Each question has 4 options, corresponding to scores of 0-3:
- 0 points: Not at all or rarely
- 1 point: Some of the time
- 2 points: A good part of the time
- 3 points: Most or all of the time
After completing the test, the system will provide a result evaluation (0-39 points) based on your total score, divided into four levels: No Depressive Symptoms (≤4 points), Mild Depressive Symptoms (5-7 points), Moderate Depressive Symptoms (8-15 points), Severe Depressive Symptoms (≥16 points). Each level provides corresponding symptom descriptions and intervention suggestions.

This questionnaire is suitable for:
